Upto 12 Months Waiting List
In order to register as NHS patient, follow these steps:
Step 1
Please complete the “New Questionnaire Form.” and email it to reception@gkdentalhawick.co.uk
Step 2
Once your form has been received, you will receive a link to complete your medical history via our online portal
Once above steps are complete, you will be added to the waiting list.
If you are in pain then you may be assessed privately and still have the option switch to NHS later on.
